Cherokee County Community Health Assessment Reports (CHA)
The North Carolina Department of Health and Human Services requires that each county conduct a Community Health Assessment (CHA) every four years. A community health assessment is a systematic collection, assembly, analysis and dissemination of information about the health of the community. Through this collaborative effort of community leaders, public health agencies, businesses, hospitals, private practitioners, and academic centers, a community assessment team works to complete this process.
This assessment or report includes information about the health of the community and is the foundation for improving and promoting the health of community members. View past reports in our Archive Center.
Community Health Improvement Plan (CHIP)
A Community Health Improvement Plan (CHIP) serves as a strategic health improvement plan for the entire county. It results from a collaborative action planning process and helps encourage cross-sector collaboration.
The goals, objectives, and strategies in a CHIP focus on priority health needs within the community, which were set during the Community Health Assessment (CHA) process. View past plans in our Archive Center.
Cherokee County State of the County Health Report (SOTCH)
During the years that a community health assessment is not completed, a State of the County's Health Report is completed to provide updated information about priority health issues specific to the county.
